Medical Alerts Can Save Lives:

What would happen if you had a medical emergency but were unable to tell the responders anything?  Perhaps a neighbor or relative could speak for you, but they aren't always available.  When a Campion Ambulance crew responds to a medical emergency, they are trained to get as much information as possible about a patient's conditions, diagnoses, recent surgeries and medications.  Unfortunately, this information is not always available, or the patient's condition requires immediate action, which does not leave much time to gather up this important information.  The medical history of a patient is extremely important because it helps the EMT's and Paramedics treat the patient, and alerts them to watch for potential complications.  Doctors and nurses rely on the information provided by the medics, to make decisions about how a patient is prioritized and treated.  Patients with a complicated medical history can be a challenge to treat if important information is not readily available.

There are many steps you can take to make sure the right information is there when it is needed.  Patients with chronic conditions such as diabetes, asthma or medication, insect and food allergies, are often advised to wear an engraved "medical alert" bracelet or necklace.  While helpful, many patients prefer to have a more inclusive summary of their health history at hand.  With the evolution of technology, some medical alert jewelry now comes equipped with built-in memory that can be read by any computer.

There are even online services that will, for a nominal fee, provide a website where you can register your medical history information.  Patients then wear a bracelet or necklace with the website address and a special access code that doctors can use to look up their up-to-date history.

Medical History Summary Form
Whether your concern is for yourself, a friend or elderly parent, we have provided a free, easy to use form that you can use.  Simply download the form, print it, fill it out with your most up-to-date information, and attach it to your refrigerator or on the inside of the door of the cabinet where you keep your medications.  Medical personnel are trained to look for such information stored in these places.

You can download and print your own form {here} or request a free copy from Campion Ambulance Service by calling (203) 753-2293. The form has instructions in both English and Spanish.

Pets:

Public shelters cannot accept pets because of state's health and safety regulations. It is very important that you make the necessary arrangements for your pets. Please visit the Animal Companion Emergency Advice section.
